Litotricia extracorpórea con ondas de choque como apoyo a la nefrolitotomÃa percutánea
Resumen
Introducción: desde la introducción de métodos mÃnimamente invasivos para el tratamiento de la litiasis urinaria, algunos pacientes reciben una mejor opción terapéutica por medio de la nefrolitotomÃa percutánea (NLP) y la litotricia extracorpórea por ondas de choque (LEC). Esta receta terapeutica, combina las ventajas de ambas técnicas. Método: el presente trabajo describe algunas caracterÃsticas de los cálculos y los resultados con la aplicación de la (LEC) en el tratamiento de los cálculos residuales (CR) después de la NLP. La muestra corresponde a 65 pacientes con CR después de una NLP, tratados en el hospital Hermanos Ameijeiras, entre abril de 2006 y diciembre de 2009, utilizando el litotritor MODULITH SLX-MX (STORZ). Resultados: las edades lÃmites fueron 28 y 74 años. Predominó el sexo masculino 55.38 %. La superficie litiásica media fue de 1,07 cm2. Los CR se localizaron con más frecuencia en cálices múltiples 32.31 % y en el cáliz inferior 24,62 %. Quedaron libres de material litiásico con una sesión de LEC, el 80 % y el 95.38 % después de tres sesiones. Después de la LEC se presentó infección urinaria (ITU), en el 13.85 % de los pacientes, quedaron con CR el 4.62 %. Conclusiones: la LEC continúa siendo una eficaz herramienta en el tratamiento de los CR después de la NLP, en especial para los fragmentos que no son accesibles al instrumental flexible, asà como cuando no se cuenta con él, y que por su tamaño, no se justificarÃa un nuevo trayecto percutáneo, o este nuevo abordaje resulta técnicamente riesgoso.

Introduction: from the introduction of minimal invasive methods for treatment of urinary lithiasis, some patients are better managed using the percutaneous nephrolithotomy (PNL) and extracorporeal shock-waves lithotripsy (ESWL) This way combines the advantages of both techniques. Methods: present paper describes some calculi features and the results from the application of ESWL in treatment of residual calculi (RC) after the PNL. Sample includes 65 patients presenting with RC after PNL treated in the «Hermanos Ameijeiras» Clinical Surgical Hospital from April 2006 to December 2009. using the lithotripter MODULITH SLX-MX (STORZ). Results: limit ages were 28 and 74 years. There was predominance of male sex 55.38 %. The mean lithiasis surface was of 1.07 cm2. The RC was more frequent located in multiples calyces 32.31 % and the lower calyx 24.62 %. The 80 was free of lithiasis with one session of ESWL and the 95.38 % after three sessions. After ESWL there was urinary tract infection (UTI), in the 13.85 % of patients remained with RC the 4.62 %. Conclusions: the ESWL continues as an effective tool in treatment of RC after PNL, especially for the fragments non-accessible with the rigid instruments, neither of flexible instruments or the not-available and due its size it not justified a new percutaneous route or a new approach is technically risky.
